DEPARTMENT: Nursing Services, Extended Care


SHIFT: Days (7:00 am - 7:30 pm)


STATUS: Part-time (24 HRS/WEEK) UNION, every other weekend & holiday


GENERAL SUMMARY: Performs duties in accordance with the mission, vision and values of HealthSource Saginaw and any applicable federal, state and/or regulatory statutes/guidelines. Responsible for assessing, planning, organizing, administering, documenting and evaluating resident care in accordance with physician's treatment plan and nursing care plan on Extended Care units. Oversees the work of non-professional nursing personnel.


PRINCIPAL RESPONSIBILITIES AND DUTIES:
1. Assists in overseeing and directing subordinate personnel in assigned unit to ensure effective, efficient and safe nursing care in accordance with established standards. Assists in maintaining continuity of care for patients/residents/clients within assigned unit.
2. Works closely with the Nurse Manager and/or Charge Nurse. Evaluates resident care needs in assigned unit, determines work priorities and makes daily work assignments. Assists in evaluating and developing monthly staffing schedules.
3. Assists in overseeing the planning of resident care through the nursing process of assessment, planning, implementation and evaluation and promotes acceptable nursing practice in assigned unit.
4. Participates and assists in direct care to the residents. Administers medications, applies and changes dressings, addresses any/all nursing care needs.
5. Implements approved policy and procedures and serves as resource to nursing assistant and unit secretary personnel.
6. Orients and trains employees within assigned unit. Assists in evaluating the work of subordinates and recommends/implements disciplinary actions when necessary.
7. Assists with the development, evaluation and presentation of in-service and educational programs. Assists with developing and coordinating nursing care conference within assigned unit.
8. Develops and maintains cooperative working relationships with variety of facility departments necessary to schedule appointments, order supplies, gather and exchange information, resolve operational problems and improve services provided.
9. Assists with the ongoing Performance Improvement program.
10. Serves on committees and attends staff meetings. Attends all mandatory in-services and maintains and enhances own personal and professional growth and development.
11. Makes daily rounds to evaluate effectiveness of care delivery and performance of subordinates to ensure compliance with Performance Improvement and regulatory standards.
12. Performs other duties as assigned.
13. Regular and punctual attendance is required.

14. Practices Standard Precautions, utilizing hand washing and barrier use when there is risk of contact with blood, body fluid, mucous membrane or non-intact skin. Provides barrier for self when there is evidence of non-intact skin on hands


KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
1. Must possess current license as a Licensed Practical Nurse from the State of Michigan, and successful completion of a course in Pharmacology for LPN's.
2. Should have interpersonal skills sufficient to establish therapeutic relationships with residents and to communicate and work effectively with residents and staff in accordance with resident care plans, facility policies and procedures and the Bargaining Agreement.
3. Should possess analytical abilities necessary to oversee and organize the work of others and must possess sound nursing knowledge necessary for sound information, implementation and documentation of nursing care plans.
4. Should possess patience, poise and tact in dealing with residents, families, visitors and facility employees.
5. The nurse must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the residents served by HealthSource Saginaw. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the resident's status and interpret the appropriate information needed to identify each patient's/resident's requirements relative to his/her age specific needs. Must be able to provide care needed as described in the department policies and procedure.
6. Approved CPR certification.
