Two different plans to choose from


Schedule: Full-time (37.5/hours week, every other weekend/holiday)

Shift: 2nd Shift (3:00 pm - 11:00 pm)

GENERAL SUMMARY: Under the direct supervision of a staff nurse provides various aspects of nursing care such as bathing, dressing, feeding, transferring, restorative rehabilitation to an assigned group of patients/patients. Maintains a safe environment by following safety standards and maintaining work area and equipment neat and clean and in working order. Maintains an open communication with the Nurse Manager/Charge Nurse, RN Coordinator, Supervisor and staff nurses and reports any findings. Documents the related aspects of care and honors the Patients' Bill of Rights. While specific duties of the individual may vary according to the level of care, the following represent duties typical of the Competency Evaluated Nursing Assistant (CENA) in a Rehabilitation setting. Performs duties in accordance with the mission, vision and values of HealthSource Saginaw and any applicable Federal, State or local regulatory statutes/guidelines.

Principal Duties & Responsibilites:

•Provides for the personal care and comfort of assigned patients including bathing, dressing, backrubs, applying lotions, transferring patients/patients to and from bed, turning patients/patients, applying, monitoring, and releasing restraints, using special devices such as splints and providing supervision.
• Assist patients/patients with mealtime preparation consisting of passing food trays, setting up, assisting patients with specific tasks or feeding them. Transports patients to and from day room for meals. Provides supervision for those patients at risk for aspiration.
•Performs basic nursing care procedures: Temperature, pulse, respiration, blood pressure, weight, ROM, intake/output measurements, bladder scans, accuchecks and recording data on specified documents following prescribed Functional Independence Measure (FIM) guidelines.
•Implements bowel and bladder programs. Assists patients to and from bathroom or commode. Provides assistance to patients as to the use of bedpans or urinals. Changes urinary collection devices and empties colostomies. Administers enemas, gives suppositories as directed by the nurse. Assists patients with self catheterization programs
•Prepares patients to be taken to treatment by assisting them to stretcher or wheelchair. This procedure may also include transporting them to any designated activity in a timely manner, only if a pervious arrangement has not been made.
•Maintains patient's rooms in a neat and clean manner including changing beds, putting clothes in the appropriate areas, checking equipment and other duties that may be designated by licensed staff.
•Collects routine urine specimens or other non-routine specimens that may be directed by the staff nurse (i.e. stool, sputum).
•Assists with teaching patients with personal care functions while they are performed as directed by O.T.in order to enhance the rehabilitation process.
•Performs and assists patients with ROM and other restorative rehabilitative procedures.
•Participates with the patient's care planning process and follows the care plan to deliver the care for expected outcomes.
•Practices Standard Precautions, utilizing hand washing and barrier use when there is risk of contact with blood, body fluid, mucous membrane or non-intact skin. Provides barrier for self when there is evidence of non-intact skin on hands.


Knowledge, Skills and Abilities:
•Must have 6 months to 1 year of acute care experience.
•Certificate of completion from an approved N.A. training program.
•Competency based State of Michigan Certificate is REQUIRED.
•Completion of a high school level of education.
•Excellent interpersonal skills to deal effectively with residents with physical and emotional problems.
•Proven history of excellent attendance and reliability.
