Shift: Days – flexible hours for days, nights, and weekends

GENERAL SUMMARY: Under the direction of the Rehabilitation Executive, and in accordance with the policies, procedures, mission, vision and strategic plan of the Rehabilitation Center, is responsible for providing psychosocial services to the patients in the Rehabilitation Center. Plans and implements education/support programs to help the Rehabilitation Center patients and their support systems to understand illness and disabilities and the process of resolving complex personal, financial and environmental difficulties which affect their daily lives. Provides consultation to the interdisciplinary Rehabilitation team regarding the impact of psychosocial factors on patients' care and assists patient and families with discharge planning. Acts as a resource and clinical monitor to the social workers in the Extended Care program. Performs duties in accordance with the mission, vision and values of HealthSource Saginaw and any applicable federal, state and/or regulatory statutes/guidelines.


  • Consistent with the stated mission, vision, values and strategic plan of the Rehabilitation Center and HealthSource Saginaw, develops and implements long and short term goals to meet the psychosocial needs of the patients in the Rehabilitation Center.
  • Assures that the Rehabilitation Social Work functions are in compliance with all regulatory/accrediting requirements, including CARF.
  • Works in collaboration with Case Management to facilitate patient discharges, including placement in other facilities, arranging home care and equipment and working with community agencies to mobilize service in the patient's behalf.
  • Participates in the planning and implementation of the Quality Management program of the Rehabilitation Center. Participates in the HealthSource Saginaw Quality Management program and demonstrates quality through the work he/she performs.
  • Assesses the need for staff development programs in relation to psychosocial needs of the rehabilitation patients and their support systems.
  • Provides direct psychosocial casework services and counseling to patients and families in relation to emotional problems, including problems of anxiety and stress precipitated by illness, disability, and hospitalization.
  • Interprets social, psychological, emotional and family problems for the interdisciplinary team by:
    • Participating in rehabilitation team conferences.
    • Consulting individually with team members.
    • Plans, implements and evaluates support groups/educational needs of patients.
  • Maintains professional knowledge through participation with continuing education, conferences, workshops, etc. Keeps current on State and Federal regulations, as well as professional standards and make recommendations, as appropriate, concerning changes needed in policy and procedure.
  • Performs additional assignments as requested by the Executive for Rehabilitation Services or as may be required by changes in current Federal, state or Regulatory Agency requirements/guidelines.


  • Must possess levels of knowledge of psychosocial therapies and social casework generally acquired through completion of a Master's Degree from an Accredited School of Social Work (MSW).
  • Must be licensed, or eligible for licensure, by the State of Michigan.
  • Requires a high level of analytical abilities in order to asses patient needs, develop associated treatment modalities, and provide effective counseling in resolving complex cases encountered with patient caseload.
  • Work requires a high level of interpersonal skills to provide counseling to families and patients in time of great stress and to relate well with other facility personnel who also work under stress.
  • Work requires regular exposure to mental strain (about 60% of work time) when providing counseling to patients/families.
  • The MSW must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in the therapy setting. He/she must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status. He/she should be able to interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs. Must be able to provide care needed as described in the department policies and procedures.
  • Work requires knowledge of and expertise in management techniques to effectively understand the roles of Social Workers in Rehabilitation and Extended Care.
